Current (Valid) Certificate of Good Conduct from DCI

Medical Letter
Medical fitness letter from a Government Hospital

Chief's Letter
Letter from Local Chief (Home Location) with official stamp and telephone number

Three Referees
Letters from 3 referees with copies of their ID cards attached. One must be a close relative

KRA PIN
Copy of KRA PIN certificate

NSSF Card
Copy of NSSF card

SHA Card
Copy of SHA card (Social Health Authority)

Passport Photos
Two colored passport size photos

Birth Certificate
Original birth certificate (Optional)
